Hand In Hand Program
Each year more than 50,000 children die in the United States; 75% occur in a hospital setting. Pediatric palliative care seeks to support decision making, provides interventions to minimize suffering while empowering patients and their families, and facilitates care in the hospital or home to children with life limiting or life threatening illnesses.
The Hand in Hand Program at Children’s is an inpatient palliative care service which provides consultation and support to the patient’s primary treatment team. The Hand in Hand Program addresses the physical, emotional, social and spiritual needs of children with life limiting or life threatening illnesses and their families.
The Hand in Hand Program seeks to prevent and relieve distress and support the best possible quality of life for such patients and their families preferably from the time of diagnosis through family bereavement. The Program assists the patient and family in developing a comprehensive plan of care which is modified as needed according to the changing medical status, patient and family goals and needs.
The Hand in Hand Program consists of a multidisciplinary team experienced and trained in pediatric palliative care. The team includes a medical director, program coordinator/social worker, nurse case manager, chaplain, and child life specialist. Children’s Hospital staff are:
- Andrew Macfadyen, MD, Hand in Hand Medical Director
- Sue Bace, LCSW, Hand in Hand Coordinator
- Sheila Mee, DMin, BCC, Chaplain
- Cheryl Darnall, RN/BC, Nurse Case Manager
- Nicole Walker, Child Life Specialist
The Hand in Hand Program is also a resource to the hospital, community and region on pediatric palliative care. The Program provides education to hospital staff on an ongoing basis to ensure that an accurate understanding of palliative care within the organization occurs and is maintained.
